Promoting communitybasedrehabilitationThe model community that the City strivesto be is“a city where residents help eachother to live continuously with vitality, ingood health, and without anxiety,”and theCity is working hard to realize this objective.Under the“community-based rehabilitation”concept conceived by the City, avariety of organizations and people associatedwith community living are workingtogether to establish a system in whichresidents can receive the necessary supportscontinuously and systematicallyinside the community. The Regional Support Centeroffers activities, including programs designed to prevent seniorpeople from requiring“long-term care”certification as well ascomprehensive consultation assistance, advocacy programs,and supports for care managers. The Center also managesIn-home Care Support Centers that are situated in six locationsthroughout the City.Creating a community medical networkThe City, the Japanese Red Cross Musashino Hospital, and theMusashino Medical Association are working together to develop amedical network that will enable residents to continue receiving medicalcare at their homes around the clock without anxiety. The CommunityMedical Partnership Forum held every year is organized with thecooperation of the three stakeholders in order to spread and raisecitizens’awareness of the collaborative efforts being undertaken byhospitals and clinics as well as the role of the community. In 2008, theStroke Community Partnership Pass was started. The three stakeholdersare collaborating to establish a system to seamlessly provide medicaland care services to stroke patients.Community Medical Partnership Forum31
